It’s one of the most common types of injuries presented at Lee Memorial Health System Wound Care. Health experts say diabetic foot ulcers are difficult to prevent and treat since most patients can’t feel the wound. Dr. Jeremey Tamir, wound care medical director, says about 95 percent of diabetic foot ulcers are caused by lack of nerve sensation. “No pain, no protection. No protection means damage.” Dr. Tamir says high glucose levels over a long period of time can cause nerve damage to the patient’s feet. “They may walk on a nail, and we’ve seen that many times. The nail will be stuck in their foot for a few weeks until a horrible infection happens and they don’t know about that.” He says to treat the problem patients are advised to remove pressure from the wound. Things a patient should notice are redness, swelling, odor and firmness around the wound. “Our feet carry the weight of our body, so they are constantly hammered every step we take. It’s alike a mallet on our feet.” In order for the wound to heal, he says it’s vital for the patient to comply with doctor’s orders and stay off the wound. “We need to train them. We need to build a special custom made shoe for them. They need to look at the feet every day, and if there’s a blister or redness they need to stop walking.” Patients who develop diabetic foot ulcers need to know their limits and avoid walking barefoot. “There are other ways that will help those patients. We can do all types of surgical procedures that will remove the boney prominence from under the skin where you put weight on so there will be less weight on the area. It’s surgical offloading procedures which will eliminate the patient’s need to be so compliant,” said Dr. Tamir. Other times the patient may have to spend a few weeks in the hospital elevating and resting their foot so it can heal.
