The Community Led Prevention Team at TPHC hosted a webinar to showcase where personalised care roles are being used in secondary care to tackle health inequalities and to demonstrate the impact on patients, communities and healthcare services. London are leading the way in expanding and embedding personalised care support into secondary care and many projects are already seeing huge impacts. The session welcomed a fantastic group of speakers and panellists who shared learnings from services using personalised care roles to support secondary care services. Jane Clegg, Regional Chief Nurse and SRO for Personalisation at NHSE London presented an ambition for London that ‘Every Hospital Trust in London has access to personalised care roles (e.g. Social Prescribing Link Workers, Health & Wellbeing Coaches, Care Coordinators) by March 2025’.
