The sural nerve is small but very important and it can be damaged by varicose vein treatment. In this video I show you where to find it on a duplex ultrasound scan. We can see the saphenous vein in the middle of the picture and when I gently press we can see that the vein almost “winks” at us indicating that it is compressible and that is one of the cardinal features of a vein. To the right of it you can see the sural nerve - we are approximately – we are approximately ten centimetres above the ankle bone. And we note that the small saphenous vein is in direct contact with the sural nerve. So that would be a place where the vein and the nerve are in direct contact and that is an area where I think it would be inappropriate to cannulate the vein. As we move the probe slightly more proximal you can see on the screen that the nerve and vein are now separated and that would be a much more appropriate place to cannulate the vein. So once more, sliding the ultrasound probe down we see the vein and nerve come into direct contact there and sliding it up we see the vein and the nerve are separated by a distance of approximately one centimetre.
