Join us in this lecture where Professor Zach Murphy will be discussing the facial nerve, cranial nerve VII, focusing on its detailed anatomy, diverse functions, and clinical correlations. We begin with its origin from the pontomedullary junction and trace its intracranial course through the internal acoustic meatus alongside the vestibulocochlear nerve. From there, we examine its passage through the facial canal of the temporal bone, its key branches (greater petrosal nerve, nerve to stapedius, and chorda tympani), and its exit via the stylomastoid foramen before branching within the parotid gland. We explore CN VII's mixed sensory, motor, and parasympathetic roles. Motor innervation controls the muscles of facial expression, the stapedius muscle in the middle ear, and the posterior belly of the digastric. Parasympathetic fibers provide secretomotor supply to the lacrimal, submandibular, and sublingual glands. Special sensory fibers carry taste from the anterior two-thirds of the tongue via the chorda tympani, while general sensory fibers innervate part of the external ear. Clinical discussion covers the hallmark features of a lower motor neuron lesion, such as Bell’s palsy, where both the upper and lower facial muscles are affected on one side, versus an upper motor neuron lesion from a cortical stroke, where forehead sparing occurs due to bilateral cortical innervation. We also touch on associated symptoms like hyperacusis, loss of lacrimation, reduced salivation, and ageusia, depending on the lesion location along the nerve’s pathway.
