Insertional Achilles tendinosis - The open approach.

In this first video we go over an open approach to insertional Achilles tendinosis. We talk about different fixation techniques and surgical approaches. Insertional Achilles tendinosis refers to a condition characterized by chronic degeneration and inflammation of the Achilles tendon at its insertion point on the heel bone (calcaneus). It typically involves the lower portion of the tendon where it attaches to the calcaneus, causing pain and discomfort in the heel area. When conservative treatments such as rest, physical therapy, and orthotics fail to alleviate the symptoms, surgical intervention may be considered. One surgical treatment option for insertional Achilles tendinosis is open debridement, which involves the removal of damaged or diseased tissue from the tendon. Debridement aims to eliminate the sources of pain and inflammation and promote healing. In addition to debridement, calcaneal ostectomy may be performed as part of the surgical treatment for insertional Achilles tendinosis. Calcaneal ostectomy involves the removal of a portion of the calcaneus (heel bone) to address issues such as bone spurs or calcifications that may contribute to the condition, essentially reshaping the calcaneus to create a smooth area of attachment of the achilles. By removing the bony prominence or irregularities, this procedure aims to relieve pressure and reduce irritation on the tendon insertion. It's worth noting that surgical treatment options may vary depending on the specific case and the severity of the condition. Surgical techniques described for insertional Achilles tendinosis include lateral release, medial release, central tendon splitting (CTS), and complete detachment and repair as we describe here. Additionally, studies have reported two categories of surgical treatment: debridement alone or debridement with augmentation in cases of excessive tendon loss. It is important to consult with a qualified orthopedic specialist or foot and ankle surgeon to determine the most appropriate treatment approach for insertional Achilles tendinosis, including the consideration of open debridement and calcaneal ostectomy. They will be able to evaluate your specific condition and provide personalized recommendations based on the best available evidence and their clinical expertise.
