This is a 60 year old woman who has been referred with a fixed, dilated atonic pupil that transilluminates after recovering from endophthalmitis. There is a radial tear in the posterior capsule and vitreous is prolapsed through and wrapping around the inferior haptic which fortunately is in the bag and covered by anterior capsule. The iris is fused to the anterior capsule in this region as well and she has significant inflammatory debris in the vitreous and precipitates on the cornea endothelium. We have made a decision to clean up the vitreous debris and adhesions, free up the iris 360 and do an iris cerclage to create a better pupil aperture and reduce her astigmatism with a paired LRI.
