Here we show a case from a few years ago. I think that this case was with my good friend Andrew Doan who started EyeRounds and medrounds and is very active in the AAO young ophthalmology group. This video is part of an ebook found on http://medrounds.org/cataract-surgery.... We have a tear in the posterior capsule and cannot place our usual single-piece acrylic in the bag. We are placing an MA 50 (Alcon) acrylic 3 piece IOL in the suclus using the moarch injector system (B cartridge). The video shows how to keep the IOL right side up as the haptics are placed in the sulcus. Often the sulcus is not perfect across from the wound as this is the area that the rhexis goes radial, the phacio hits the capsule, and the chopper tears the capsule so it can be tricky. If the sulcus is not great across from the wound you can place the haptic in the anterior chamber initially, place the trailing haptic in the sulcus and then later place the leading haptic in the sulcus with a sinsky hook. Of course, it is never a good idea to place a single piece acrylic in the sulcus as it can lead to pigment dispersion, glaucoma, uveitis, and hyphema.
