#WoundHealing #AcuteWounds #ChronicWounds #Biofilms #WoundBedPreparation #TIMEFramework #Healthcare Drawing on "Wound Care Essentials", this video explores the complex biological process of wound healing. Acute wounds follow a predictable, dynamic four-phase cascade: Hemostasis (stopping bleeding via platelet aggregation and clot formation), Inflammation (clearing debris, preventing infection with immune cells, coordinated by cytokines), Proliferation (forming granulation tissue, angiogenesis, re-epithelialization, and wound contraction), and Maturation/Remodeling (strengthening the new tissue matrix, forming a scar that is less strong than original skin). Enzymes like MMPs are crucial throughout for breaking down and building tissue. Chronic wounds deviate from this timeline, failing to heal within expected periods. They are often linked to underlying health issues and characterised by prolonged inflammation, altered wound fluid inhibiting cell function, and barriers like low-level contamination or impaired skin cell migration. Host factors such as age, comorbidities (e.g., diabetes, poor circulation), weakened immune systems, poor nutrition, and smoking significantly hinder healing. Biofilms, organised bacterial communities within a protective matrix, are a major challenge in chronic wounds. They trigger damaging inflammation and resist antibiotics/antiseptics. Management involves physical removal (debridement), cleansing with antiseptics, and suitable dressings. Effective wound care requires preparing the wound bed, often using the TIME framework: managing non-viable **T**issue, controlling **I**nfection/**I**nflammation, achieving **M**oisture balance, and assessing **E**dges. Chronic wounds also impact patients' overall well-being, frequently causing clusters of symptoms like pain, fatigue, anxiety, and depression. Comprehensive assessment and adaptive treatment plans are essential for effective management.
