😕 Big erosion in the base of penis

Hello dear patients. Today I would like to give an example of big erosion in the base of penis. A man came to us, his job is a taxi driver and he has a relatively large number of sexual contacts, he doesn’t have them every day of course, but still he has quite many sexual partners, including casual partners. Several days ago he had this big erosion appeared on glans penis. He uses a condom, that’s why he did not have any discharges etc., however, this erosion bothers him. I would say that erosion of such size, this erosion was about 2 cms in length, is very rarely met on penis. So, here in the beginning, we thought mostly about streptoderma, streptococcal impetigo. The patient did not use any medications before coming to us and together with culture test for bacteria, we took analysis from the surface of ulcer and from inside the canal, for sexually transmitted infections and I’ll remind you that during erosion and ulcers, it’s mandatory to include treponema pallidum which is a causative agent for syphilis, as well as herpes of first, second and zoster types into such analyses. In some cases I even found cytomegalovirus, however strange this may seem. I cannot tell you whether it was related to ulcers or whether these were casual artifical findings. However, I would advice to check for the whole family of herpes viruses. So, treponema pallidum was positive, which indicates a fresh infection of syphilis, hard chancre as in full complex of analyses, only total antibodies were present in the blood. I would like to mention this case as a case of a chancre which is quite big in its diameter, erosive chancre. It is quite non-dense on palpation. It’s written that it should be dense on palpation, however, it’s not the case, there are different cases in different times in different people after using a condom. This can be in the base of penis as in our case. I would like to note that condom does not always save from sexually transmitted infections and chancre can be localized in the base of penis as well as condyloma acuminata. If the doctor does not think about this disease and if the patient was only checked for RPR reaction, that is if treponema was not found, then it would lead to appearance of secondary syphilis with more severe consequences. This patient was not treated by us, he went elsewhere for treatment, however, he was set this diagnosis and there was quite a big and erosive chancre. So, if you have any erosions, ulcers or what you think as rubbings, then they have to be checked.
