Bilateral vocal cord palsy in stridor : Laser cordotomy to improve airway

Stridor is a clinical condition, where any structural blockage (tumour, oedema or B/L vocal cord palsy) causes narrowing of the airway. It is a medical emergency and the standard approach is to bypass the obstruction by placing the tube below the obstruction i.e, tracheostomy. It is a safe procedure and ideal surgery to perform in such situations. In situations of B/L Vocal cord palsy as the reason for stridor, we can improve airway by opening up the fixed vocal cord in the posterior part. This procedure is case posterior cordotomy to improve airway. In this video we share our experience in managing once such case. We were able to avoid a tracheostomy with this technique. Eventhough, this improves the breathing tube it compromises the voice output. They will be able to speak after the procedure but the volume of sound will be low pitch and raspy.
