Recorded on 11/20/2021 52-year-old female with history of T3N0 distal rectal adenocarcinoma status post chemoradiation, surgery and adjuvant capecitabine now with recurrence on CT imaging 14 months after completion of treatment. Summary: The panel suggests obtaining an MRI, a biopsy to confirm recurrence, and tumor molecular markers be obtained to help determine systemic options. The panel recommends induction chemotherapy followed by either resection, consolidative re-irradiation with concurrent chemotherapy, or chemoradiation followed by resection depending on tumor response. Proton radiation therapy could provide benefit if available In terms of systemic therapy options, the panel suggests FOLFORINOX or FOLFOXIRI. However, the patient is at high risk for cytopenia, and the panel would suggest using a prophylactic growth factor. If the tumor markers return with RAS/RAF-wild type, the panel would consider adding an anti-TGFR option. If the tumor markers return with RAS-mutant type, then the panel suggests the addition of bevacizumab.
