Transcript: peyronie's disease affects almost 13 percent of american men peyronie's disease is a condition that affects the erectile function of men it is the noticeable lump bump or plaque on the inside of the penis that may cause curvature that is bothersome to patients or their partners peyronie's disease is thought to be a disorder of wound healing meaning that there is some sort of microscopic one-time or repetitive trauma to the penis likely during intercourse combined with an excess deposit of collagen that results in a scar this scar tissue is on the surface of the erectile tissue of the penis and it is very inelastic so it affects the penis in many ways in terms of shortening curvature with erections difficulty achieving and maintaining erections and patient and partner discomfort peyronie's disease is thought to be caused by single or repetitive microscopic trauma to the penile tissue this results in a scar that is hard to make and hard to break this scar is formed by an abnormal wound healing process combined with a genetic predisposition for excess collagen deposits in the penile tissue this results in a palpable scar lump bump or curve to your erections that can cause a problem with intercourse peyronie's disease and the curve that it may cause with erections can impact intercourse in several ways patients may notice shortening of the penis they may have pain with erections their curve directions may cause problems with positioning or discomfort in their partners patients may also find it more difficult to achieve or maintain erections that are affected by peyronie's disease peyronie's disease may be diagnosed in your primary care office if your provider is familiar with the disease state condition otherwise it's diagnosed in my office by physical exam examination of any photos you or your partner may have of your curve directions as well as performing a penile duplex doppler ultrasound to assess your blood flow and the amount of curvature wasting narrowing or any other concerns that peyronie's disease may cause with the function of your erections the american urologic association established disease state treatment guidelines in 2015 for peyronie's disease there is only one fda approved treatment for peyronie's it is called clostridium collagenase or xyaflex it is an injectable treatment in the office aimed at breaking apart the collagen deposits that cause the peroneus plaque any other treatment oral topical is off label there is also surgical treatment of peyronie's disease a plication to straighten out your curvature or in some cases a penile implant which may return your normal erectile function as well as changing or straightening your curvature the most appropriate and best treatment option for you and your peyronie's disease is determined in my office we determine this after an assessment of your erectile function and the curvature with an in-office assessment of your erections the blood flow and the curvature measurement by penile duplex doppler ultrasound there may be several consequences for men with untreated peyronie's disease these men may continue to see decreases in length and girth of their penile measurements they may see continued erectile dysfunction in problems achieving and maintaining erections their partners may have continued discomfort with intercourse we know that approximately only 12 percent of men improve spontaneously without any treatment of peyronie's disease that is why it is so important to speak with a urologist and a specialist in terms of your peyronies [Music]
