Median duration between diagnosis and ovariohysterectomy was significantly shorter for dogs treated at referral hospitals (0 days) versus community clinics (1.0 day); however, delay was not related to survival to hospital discharge. Duration of hospitalization did not impact survival to hospital discharge nor survival for at least 1 week after surgery. Although private practices estimated that economic euthanasia is the most likely alternative outcome for 13% of patients unable to access care at their practice, 80% would be reluctant to refer to a high-quality, high-volume spay-neuter clinic. The three primary reasons indicated by private practices with a reluctance to refer were a perceived lack of prompt appointment availability (20%), lack of aftercare (18%), and lack of adequate equipment (14%). In contrast, most high-quality, high-volume spay-neuter clinics estimated that they generally accept referrals the same day (50%) or the next day (30%) and 95% of high-quality, high-volume spay-neuter clinics reported having the resources needed to treat most pyometras. 100 There are several opportunities for partnership between private practices & high-quality, high-volume spay-neuter clinics in the treatment of pyometra. The concerns expressed by private practices regarding referral to high-quality, high-volume spay-neuter clinics were not shared by high-quality, high-volume spay-neuter clinics or could be ameliorated by referral to an high-quality, high-volume spay-neuter clinic for surgery & transfer back to the PP for aftercare. 76, 100 Prevention 73 Surgical sterilization via ovariohysterectomy and ovariectomy is preventative of pyometra. Should signs of estrus or development of pyometra/stump pyometra occur post-preventative surgery, then possible causes include ovarian remnant syndrome, ectopic ovarian tissue, exogenous progesterone or estrogen administration, neoplasia of the endocrine system, or unethical veterinarian behavior. Progesterone-related diabetes mellitus 74 Progesterone-related diabetes mellitus in dogs is known for its particular potential for diabetes remission. Progesterone-related diabetes mellitus is recognized due to diestrus, exogenous progestogen exposure, pregnancy, and progesterone-production ovarian dysfunction. Pyometra represents additional inflammatory and septic negative influence on insulin sensitivity, and its diagnosis associated with canine diabetes mellitus is therapeutically challenging. The estrus cycle's hormone fluctuations seem to modulate peripheric insulin sensibility by influencing insulin receptor affinity and its binding capacity, as well as modulating tyrosine kinase activity. Pyometra was shown to negatively influence insulin receptor compensatory mechanisms to insulin resistance causing glucose intolerance. Spaying and pregnancy termination may cause diabetes remission in progesterone-related diabetes mellitus diagnosis cases in a median time of 10 days. Pharmacological annulment of progesterone effects may benefit patients unable to undergo surgery; however, remission chances are virtually null. The ALIVE (Agreeing Language in Veterinary Endocrinology) project proposed new criteria for canine diabetes mellitus diagnoses and subclinical diabetes recognition. These new concepts may increase the frequency of a progesterone-related diabetes mellitus diagnosis and, even more, its relevance. Spaying represents a preventive measure against pyometra and progesterone-related diabetes mellitus that should be individually assessed in light of its recognized benefits and harms. Pyometra in devleophing countries 75 In developing countries such as Uganda, domestic dogs suffer high burdens of infectious diseases often with high mortalities. At of the dogs that died, 50% were parvovirus infection cases while other conditions included babesiosis (13%), poisoning (7.8%), pyometra (7.8%) and liver dysfunction (5.1%). Thus preventable diseases were the most frequent reasons for morbidity and mortality of puppies in Uganda.
