In COPD there is a weakness of the alveoli. This is a fibrotic process and there is a rupture of the two alveoli and this becomes a big bulla. There are multiple bulla and this is one of the commonest pathology in the CT findings of the COPD. When the person is having this bulla they are not really working good for the lungs. They will compress the normal working lung and the lung is not able to work properly. That time you have to do the bullectomy, i.e. you have to remove the bulla so that we allow the other lungs to work properly. Second thing is called the lung volume reduction surgery. In that you try to remove the bulla by stappling the bulla so that you can reduce the lung. Because of teh continuous retention of the abcess the lungs will be very big. When you remove this one the lungs will come back to its original shape and they will regain some extent of the elasticity power so that they are able to get the oxygen properly. So this is the lung volume reduction surgery.
