Intraventricular tumors are rare lesions and comprise different pathological entities such as colloid cysts, subependymomas, central neurocytomas, subependymal giant cell astrocytomas (SEGA), choroid plexus tumors, meningiomas etc. Intraventricular tumors are generally benign lesions and the treatment of choice is neurosurgical resection, which can be challenging due to their location. Different surgical approaches to the ventricles are described. Regarding the fourth ventricle, approaches as the transvermian, a subtonsillar-transcerebellomedullary, a superior transvelar approach or also endoscopic approaches were described. The transvermian approach has been performed frequently in history, but data showed high rates of cerebellar mutism and disequilibrium. To enter the lateral and/or third ventricle a variety of approaches including the frontal-transcortical, the anterior/posterior interhemispheric-transcallosal and the contralateral interhemispheric-transfalcine-transprecuneal approach have been described.
