Welcome to Symbiosis University Hospital and Research Centre! Our doctors explain a recent high-risk case. A 14-year-old female patient presented to our facility with quadriplegia persisting for one month, hailing from Buldhana. She had a history of neck pain, progressively worsening over the past two months, with bilateral upper and lower limb symptoms. Previous consultations with local neurosurgeons and diagnostic imaging (X-rays, MRI) indicated C1-C2 TB spine with significant cord compression. Despite seeking opinions from various hospitals and physicians, the family remained unsatisfied until they were informed about our facility, Suhrc, through relatives. Upon admission, the patient exhibited complete limb immobility and urinary incontinence. Repeat imaging, including MRI and CT scans of the cervical spine, was conducted to reassess the condition. Following a comprehensive evaluation, the patient and her family were briefed on the disease's nature, guarded prognosis, and the high surgical risk involved. A multidisciplinary approach was adopted, involving spine surgery (Dr. Alok Gadkari, Dr. Bharati Sharma, Dr. Yogesh Narkhede), respiratory medicine (TB specialist, Dr. Meenakshi Bhakare), anesthesia (Dr. Niteen Bhalme), and skull base surgery (Dr. Gaurav Chamania, Brig (Dr.) Ramakrishnan) teams. The surgical plan comprised posterior decompression with fixation using screws and rods, addressing spinal cord compression, along with anterior endoscopic decompression of infected tissues via nasal endoscopy. The procedure, undertaken with explicit consent from the patient and her family, lasted four hours and concluded with successful extubation and transfer to the ICU. Postoperatively, the patient exhibited immediate improvement in limb strength. By the fifth day, she was fitted with a custom-sized cervical collar and initiated into a standing regimen. Intraoperative samples confirmed multidrug-resistant tuberculosis (MDR-TB), prompting initiation of anti-TB medication per government protocols. This surgical intervention was performed at a recently established dedicated spine clinic at SUHRC.
