An ovarian cyst is essentially a fluid-filled sac or a “bubble of water” inside the ovary. These cysts are typically lined with ovarian tissue. Simple Ovarian Cysts: These are the most common type and contain only fluid. Complex Cysts: Some cysts may also have solid components, such as multiple chambers (septa) or abnormal cell linings. What is an Ovarian Mass? An ovarian mass or lump is different from a cyst because it is solid, without a fluid-filled cavity. Cause: Ovarian masses develop due to abnormal multiplication of cells, forming a solid growth. Key Difference: While a cyst contains fluid, a mass is composed entirely of solid tissue. Symptoms: Ovarian masses can present similar symptoms to cysts, such as lower abdominal pain, bloating, frequent urination, or difficulty with bowel movements. How Do We Differentiate Between an Ovarian Cyst and a Mass? Imaging Tests on Ovaries Ultrasound: Both abdominal ultrasound and transvaginal ultrasound are critical in identifying whether a growth is a cyst or a mass. MRI or CT Scan: These advanced imaging techniques help determine the composition of the growth and assess any associated abnormalities. Tumor Markers to Diagnose Ovarian Abnormality For certain patients, we measure tumor markers like CA-125 or germ cell tumor markers (AFP, Beta HCG, LDH). These tests help determine whether the growth is likely benign or malignant. MRI results combined with tumor marker findings provide a clearer picture of the growth’s nature. Frozen Section Biopsy During Ovarian Surgery In cases where imaging and markers are inconclusive, surgery may be recommended. During surgery, a frozen section biopsy can help identify whether the growth is cancerous. When is Surgery Necessary For Ovarian Cysts & masses? Simple Cysts: Surgery is rarely needed unless the cyst is larger than 5 cm, persists for months, or shows no signs of shrinking. Large cysts carry risks of rupture and may require surgical removal to prevent complications. Complex Masses: Ovarian masses with solid components, irregular walls, or abnormal findings on imaging or tumor markers often require surgical intervention to rule out cancer. Importance of Early Detection In many cases, ovarian cysts and masses are non-cancerous and do not require immediate surgery. However, regular monitoring through ultrasounds and follow-ups is crucial to detect changes early. When symptoms like persistent pelvic pain, menstrual irregularities, or infertility occur, consulting a specialist promptly is essential.
