Video transcript: Hi, my name is Stacey Lynema. I'm a physician assistant here at Pacific Dermatology & Cosmetic Center. We're here to talk about acne scars and acne scar treatment. True acne scars are either elevations or depressions in the skin as a result of acne. But often times I see patients who think they have scarring but it's really a phenomenon called "post-inflammatory hyperpigmentation", which is a discoloration of the skin left behind after old acne heals and that's different. It's still treatable but it's treated in different ways than true acne scars are treated. All scars come from a form of injury and so acne is a form of injury and after that happens the body lays down collagen fibers which are not as smooth as the normal tissue that was there before. The best treatment for acne scars really depends on the type of acne scarring you have, your skin type and lifestyle. For a fair skinned young male for example, who has deep pitted scars on the cheeks, treatment options include Fraxel which is a resurfacing laser or Infini which is microneedling with radiofrequency. Fraxel's good for acne scarring, it's good for brown spots, it's good for resurfacing and pour tightening as well. I would recommend Fraxel for most people with lighter or fair skin, anybody who has deeper pitted scars and anybody who wants some resurfacing for pigmentation. Fraxel oftentimes if patients have more of a fair to medium skin, sometimes we will recommend hydroquinone which is a fading cream prior to the procedure. Strict SPF before and after of course. And making sure they're complying with their acne regimen. If your a woman who's middle-aged, for example, with darker or medium to darker skin, I think that the best treatment might be Infini. It's newer, it's microneedling with radio frequency delivered down to the dermis to stimulate collagen regrowth and resurfacing. So it's both helpful for people with deep-pitted acne scars and those patients who have maybe tried some other acne treatments in the past possibly like Fraxel and haven't seen the improvement they've desired. Sometimes people need several treatments and maybe they don't realize that or were never told that and sometimes they need several treatments at shorter intervals. So sometimes I've had patients who come to me and said, "You know I had one laser resurfacing procedure three years ago and it didn't work so what should I do?" They don't recall the laser name, they say they only had one treatment and I explain to them, well really for the type of acne scarring you have and the severity of it, you would likely need three to four treatments and we should space them closer, like four to six weeks apart. The only thing that would prevent somebody from immediate treatment with acne is if they were very very tan or if they have any kind of open infection or skin condition going on where the acne scars are. I would tell an adult with acne scars not to lose hope. There are several treatments out there for acne scarring and there are always new treatments coming out. And I would also explain to them that it's important for them to seek the advice of a Dermatology provider rather than spending time and money on over-the-counter creams which likely won't provide much benefit. Unlike treatment of acne itself, treatment for acne scarring is considered cosmetic and is not covered by insurance.
