John, a 3 years old child was eating grape and started choking and was unresponsive. His mom has called an ambulance and it is en route. CPR is on-going and is going to arrive in 2 minutes. You need to discuss with your assistance and prepare your calculations prior to arrival. MARK SHEET Introduces self to team -Confirms skills and experience of nurse and junior doctor Delegates tasks Calls for paediatric cardiac arrest team Performs correct calculations Confirms cardiac arrest Commences cardiopulmonary resuscitation with correct ratio of compressions to ventilations Ensures defibrillator is attached (with pads correctly placed) Ensures intravenous or intraosseous access When anaesthetist arrives (if help called for) asks for child to be intubated After intubation allows asynchronous compressions and ventilations Performs rhythm check at appropriate time Recognises asystole Recommences and continues cardiopulmonary resuscitation appropriately Asks for adrenaline in correct dose Considers reversible causes Asks for blood gas if not done so earlier When rhythm changes stops and asks for pulse to be checked Recognises presence of a pulse and stops cardiopulmonary resuscitation Reassesses ABCDE Asks for full set of observations Hands patient over to paediatrician/intensivist when they arrive Makes appropriate plan for on-going care Comments on need to contact/involve parents Delegates tasks to other staff appropriately Global mark from examiner Global mark from team Examiner: Daana, Atiqah Team: Iman, Fadhirah, Lee Chin Leong
