Iris Claw Lens Explantation Combined with White Cataract and Adherent Pupillary Membrane This video shows the step-by-step technique for dealing with such a complex case of white cataract following vitrectomy and oil tamponade. The patient was highly myopic and had previous iris claw lenses implanted in both eyes (unknown model). The surgery shows the steps of removal of the iris claw lens via a scleral tunnel incision. The main tip here is to minimise any trauma when disengaging the iris claw lens. Surprisingly, I noted that there is a dense pupillary membrane, which was dissected with microscissors. There was significant zonular weakness and fibrotic anterior capsule. It was quite challenging to make the CCC, which required repeated trials with forceps, needle and finally microscissors. The CTR was quite useful in this case and helped to stabilise the compromised zonular/bag complex. It also helped to further clear the posterior capsule.
