Liver cirrhosis is a chronic condition in which the liver becomes stiff and develops multiple structural changes. This disease is irreversible and progressive. As it advances, patients experience various complications and health issues. When the liver hardens, blood flow through it becomes obstructed, leading to increased pressure in the portal vein. This condition is known as portal hypertension. Portal hypertension is a common condition in all cirrhosis patients. In response to increased pressure in the portal veins, the body forms new veins to relieve the pressure and restore blood flow. These veins are called varices. These veins can form in three locations: first, in the esophagus and stomach; second, around the anus; and third, near the navel and on the abdominal wall. Since these veins are newly formed and experience high blood pressure, the risk of rupture is significant. When varices in the esophagus or stomach rupture, the patient may vomit blood, a condition known as hematemesis. If the blood is passed through the anus, it is called melena. Let’s discuss the symptoms and signs a patient may experience when vomiting (HERE IS A WORD) blood or suffering from variceal bleeding. Due to the rupture of varices or vomiting of blood, the patient may experience an increased heart rate, low blood pressure, weakness, or headache. If the bleeding is severe and blood enters the lungs, it can cause breathing difficulties. Excessive bleeding can be life-threatening. Therefore, even if blood vomiting occurs in small amounts, it should not be ignored. In such cases, contact your transplant team immediately or take the patient to the hospital. To diagnose portal hypertension or determine the cause of blood vomiting, the doctor will perform an endoscopy. During this procedure, a ligature may be placed to close varices or abnormal blood vessels in the esophagus, stomach, or other affected areas. This procedure is known as endoscopic variceal ligation (EVL), and it helps prevent further bleeding. Friends, now let's discuss the treatment options available in case of blood vomiting. First, if the patient's blood pressure is very low or they have vomited a significant amount of blood, a blood transfusion may be required. If hemoglobin levels are low, a blood transfusion will be given. If platelet levels are low, a platelet transfusion will be administered. If INR (International Normalized Ratio) is very high, plasma transfusion may be needed. Secondly, there are specific types of injections that help reduce pressure in the portal vein and manage portal hypertension. This can also help control bleeding. So far, we have discussed the diagnosis of blood vomiting in cirrhosis patients and the available treatment options. Now, let’s talk about whether this condition can be prevented.There are special antihypertensive drugs that also target portal hypertension. These medications help lower pressure inside the portal veins, reducing the risk of variceal rupture and blood vomiting. TIPS is a surgical procedure in which a stent is inserted into the portal vein to relieve blockage. This helps restore blood flow through the portal vein and reduces pressure on the varices. The decision on whether a patient will benefit from medication or requires TIPS will be made by the treating doctor or transplant team.
