Anatomic study of unilateral cleft lip specimens reveal that on the cleft side, the transverse and alar parts of the nasalis shift downward and laterally onto the anterior surface of the maxilla from the dorsum nasi. These distorted muscle fibers terminate at the nare skin and the nasal alar cartilage beneath the skin. In biomechanical terms, the first auxiliary tension line group relating to the alar abduction degree, position of the nasal columella, and the nostril floor fullness is damaged. This procedure video illustrates a technique of nostril sill repair that restores the first auxiliary tension line group to reconstruct the nostril sill in any of 3 categories: full (slightly protuberant, soft-tissue bulge between the columella and the ala), point (the columella footplate and the ala converge to an apex showing no proper sill), and flat (no soft-tissue bulge between the columella and the ala, with the upper lip running smoothly into the vestibule).
