This video demonstrates how to assess and fit a suitable external eyelid weight to the upper eyelid in order to improve eye closure in patients with facial nerve palsy. In 2014, We prospectively studied fourteen consecutive patients with unilateral facial paralysis who were fitted with external weights at the Queen Victoria Hospital. We used double-sided adhesive tape, micropore tan tape, tincture of benzoin and medical glue G609. Outcome measures included duration of using weight, patient satisfaction and feedback. Results: The mean age of patients was 48.9 (23-72 years). The mean weight was 1.3 (1-1.6) g. Seventy one per cent found the external weight useful or very useful. The most common weight used was 1.4g (6 patients), however patients who found weights most useful were those that used 1g or 1.2g weights. Users of medical glue G609 reported better satisfaction and longer duration of weight adhesion. The mean duration of use was 11 weeks (range 1-22) with the most common (mode) being approximately 13 weeks. Most patients went on to undergo upper eyelid surgical loading with either a gold weight or platinum chain. Conclusion: We found external weights useful in the majority of patients. Among different techniques, medical glue is a simple, quick and effective method for securing external eyelid weight. It lasts longer than the other techniques and can be used together with ocular lubricants.
