Afferent Loop Syndrome (ALS) is a mechanical complication that can occur after certain types of stomach surgery, such as gastrojejunostomy or Roux-en-Y gastric bypass. It involves the obstruction of the afferent limb, which is the segment of the small intestine that carries bile, pancreatic juices, and other digestive fluids toward the stomach. Causes Surgical Complications: Issues with the anastomosis (connection between the stomach and small intestine), such as ulcers or narrowing, can lead to ALS. Structural Problems: Twisting of the afferent limb, scar tissue (adhesions), or internal hernias can cause obstruction. Tumors: Recurrent tumors at the anastomosis site can block the afferent limb. Foreign Bodies: Bezoars (masses of impacted food) can obstruct the afferent limb. Symptoms Abdominal pain, especially in the upper abdomen Nausea and fullness, particularly after eating Sudden, severe vomiting of bile-containing fluid that relieves the pain Weight loss due to pain after eating (in chronic cases) Diagnosis Clinical Evaluation: Symptoms and medical history are assessed. Imaging Studies: Abdominal CT scans, fluoroscopy, and upper GI studies can help visualize the obstruction and swollen afferent limb. Treatment Surgical Intervention: Surgery may be required to relieve the obstruction and correct the underlying issue. Endoscopic Procedures: In some cases, endoscopic techniques can be used to relieve the obstruction. Supportive Care: Managing symptoms and providing nutritional support. #AfferentLoopSyndrome
