Recorded on 4/16/2023 68-year-old female with history of occult metastatic breast cancer (TX N3c M1), luminal B, HER2-negative, with lymph node and bone metastases; status post palliative palbociclib, letrozole, and bisphosphonates with progression, followed by fulvestrant with progression, with repeat biopsy showing triple-negative breast cancer; status post capecitabine for 12 months with minor response, now with disease progression suspected but not proven Summary: It is important to confirm progression of disease, because if the patient is not progressing it is reasonable to consider continuation of capecitabine. Consider next generation sequencing and ctDNA testing (liquid biopsy) in the second-line setting to help inform treatment decisions. Germline genetic testing should be performed to evaluate whether the patient would be eligible to receive treatment with a PARP inhibitor. If progression is indeed confirmed while on capecitabine, the panelists would consider bevacizumab with paclitaxel, or alternatively liposomal doxorubicin as a single-agent. If sacituzumab-govitecan is approved this would be an option as well.
