Watch this video for a quick overview. 💥Mechanical Ventilation Basics [Full Guide] ➜ ➜ ➜ https://bit.ly/3i4m1v1 ➡️ Auto-PEEP Overview As we mentioned, auto-PEEP refers to a pressure in the alveoli at end-exhalation that is above the atmospheric pressure. It often occurs in patients with obstructive lung diseases that require a prolonged expiratory time. Some examples include patients with asthma or COPD. Because their airways are obstructed or constricted, the patient may have a hard time fully exhaling the machine-delivered tidal volume before the next ventilator breath is delivered. When this occurs, gases from each delivered tidal volume ends up being retained in the lungs. This is why it's often referred to as air trapping or breath stacking. Auto-PEEP often results in patient-ventilator dyssynchrony and can even cause a decreased cardiac output and a decreased blood pressure as well. If it goes uncorrected, it could even result in an increased peak airway pressure that leads to overdistended alveoli and barotrauma. ➡️ Auto-PEEP can be corrected by: ▪ Increasing the expiratory time ▪ Decreasing the inspiratory time ▪ Increasing the flow ➡️ Other Name for Auto-PEEP: ▪ Intrinsic PEEP ▪ Inadvertent PEEP ▪ Dynamic hyperinflation It's always a good idea to be familiar with all the different terminology that you may hear.
