APNA 2018 National Conference - Nurseforce for the Future The Australian Primary Health Care Association's conference for nurses working in primary health care Abstract - Evaluation of the Advance Project: Initiating Palliative Care and Advance Care Planning Through Training and Resources for General Practice Nurses - Josephine Clayton and Srivalli Nagarajan Advance is a free toolkit of screening and assessment tools and a training package, specifically designed to support nurses in Australian general practices to work with general practitioners to initiate advance care planning and palliative care in everyday general practice. Advance is funded by the Australian Government, endorsed by APNA and delivered by a national consortium led by HammondCare. The Advance Toolkit is officially recognised as an Accepted Clinical Resource by The Royal Australian College of General Practitioners. To date hundreds of general practice nurses have participated in the training program nationally. Participants have highly rated the quality and relevance of the resources and training to their practice and value for their patients. Advance will soon be developing complementary training and resources for general practitioners and practice managers to enable a team-based systematic approach to initiating advance care planning, and assessing and addressing palliative/supportive care needs in general practices.
