My child, affected by cerebral palsy, is now walking—a remarkable positive change. It's a testament to hope and progress, filling our hearts with joy and gratitude. Crouching frequently manifests in individuals with diplegic spastic cerebral palsy. Initially, many children affected by cerebral palsy may begin walking on tiptoes, but over time, their gait pattern may transition to a crouched stance. The development of crouch gait is influenced by various factors such as elongated gastrocsoleus muscles, tight knee flexors, planovalgus feet, and external tibial torsion. Crouching is a very common presentation in diplegic spastic cerebral palsy. Initially, most of the children affected with cerebral palsy start walking on tip of toes but with time, their gait pattern may change into crouch gait. Crouch gait develops because of multiple factors like overstretched gastrocsoleus, tight knee flexor, planovalgus feet & ext tibial torsion. Crouching should be prevented by good therapy and judicious use of a brace, hamstring spasticity by botulinum toxin but fixed contracture & torsion can only be managed by surgical intervention. Post-surgical therapy & proper brace should be continued for a long duration to prevent recurrence and increase in deformity. in adult, it also became troublesome to treat it because muscle & joint deformity became much bigger. in this video, tushar has got excellent outcomes after surgery & therapy.
