Ectopic Pregnancy | Tubal Pregnancy | Pregnancy Outside the Uterus | Dr Tehmina Rehman | #tehminasclinic #ectopicpregnancy #tubalpregnancy #pregnancy #tehminasclinic #drtehminarehman #gynaecologist Ectopic pregnancy, tubal pregnancy or pregnancy outside the uterus. A lot of you may have heard these terms somewhere at some time. Uterus is the normal site of pregnancy. When a woman gets pregnant, the baby is supposed to grow within the cavity of the uterus. Pregnancy anywhere outside the uterus is called an ectopic pregnancy. Because ectopic pregnancy is most often in the tubes that is why it also called tubal pregnancy . In these sites, the baby cannot really grow much, and can not survive. We always have to to keep tubal pregnancy in mind in early pregnancy. Because if it ruptures, it can be a severe life-threatening event. And patient’s life can be lost. So how do we find out if there is a pregnancy inside the tube or not? As soon as your pregnancy test is positive, you must go and visit your doctor. I tell my patience not to wait 1 or 2 weeks after missing the periods. Because if the pregnancy is in the tube, it can rupture within a couple of days of missing the periods. So you should come to your doctor as soon as your test is positive. Ask them to localize your pregnancy. Do an ultrasound to find out where it is. There may be a very tiny sac, maybe just 3 or 4 mm. That's OK, your next scan can be even after a month. And then you would be able to see the heartbeat as well. But pregnancy should be localised. If your test is positive and the pregnancy is not seen in the uterus. It does not mean that the pregnancy has to be outside the uterus. Then we do the blood pregnancy test. If on the blood test, the level is say more than 1500. Then we do the internal or transvaginal scan. From a good machine, for example, in the radiology department. So that we know for the show where the pregnancy is located. Because after 1500 level, the pregnancy should be visible somewhere inside or outside the uterus. So if we see in the uterus, we are all good. But if it is outside the uterus, then what's the next step? We have some options. Medical management can be offered There are drugs that can finish that tiny pregnancy inside. Sometimes these drugs don't work. In a lot of places, surgery is offered as the only option. This surgery is via the camera. Laparascopic surgery also called laser surgery. But if you've not had an early check up, and you have severe lower abdominal pain. Where you are sweating and maybe fainting or collapsing. You must ask your family to take you to the nearest hospital immediately. Because if it ruptures, the internal meeting can be very heavy. Then it is emergency surgery that is the only option. Where a large cut is given like that of a Caesarean, this is called laparotomy. And then going through that rapidly, the bleeding is controlled. Why does tubal pregnancy report pregnancy happen in some people? The egg and the sperm normally meet within the tube. A healthy tube brings the sperm and the egg together. And once the cells have fertilised, they come back to the cavity of the uterus. And grow there. If the tube is damaged for some reason by birth or by infection. Then it is not moving things well. Then the cells which are supposed to grow only after reaching the uterus. Start going within the tube. The tube cannot become big like the uterus. They grow a little and the tube raptures. So we always keep ectopic pregnancy in mind. Until we see a sac within the uterus. What is the sack in early pregnancy? It is just like a sac and baby develops later in it. So the moment the pregnancy test is positive. Immediately go to your doctor. Ask them to localise your pregnancy. If they can't see it, get the black test. If more than 15 hundred, then get a transvaginal scan on a very good machine. Something should be visible at this time within or outside the uterus. Stay safe, thank you.
