When to treat an abdominal aortic aneurysm. In this video, Jon M. Wesley, M.D., F.A.C.S., a board-certified vascular surgeon at Vascular Specialists of Central Florida, discusses how abdominal aortic aneurysms can be treated. “What we should do about an aneurysm when we discover it?” asks Dr. Wesley. “Well, depending on the size, we'll either watch it or fix it. If it is a small aneurysm, three centimeters in diameter, then we will continue to follow it periodically, and it's important because aneurysms do grow over time.” “Your surveillance may range from six months to a year to follow that aneurysm to make sure it has not gotten to that threshold of five centimeters in diameter. Patients who experience rapid growth in their aneurysm, whether it's five centimeters or not, may be considered for early treatment.” “When we discover an aneurysm, the first thing we do is make sure that we stop all the risk factors that are associated with growth, and probably the most important one is smoking,” Dr. Wesley continues. “Because it's associated with other conditions, we have to make sure that those conditions are well controlled including your cholesterol levels, plaque disease in your heart, arteries, and things of that nature. It is okay to exercise, dance, garden and do all of the things that you would normally do when you have an abdominal aortic aneurysm because those things are not associated with an increased risk of rupture.” “Once it has gotten to the point that it requires repair, then the treatment is essentially exclusion of blood flow into the aneurysm sac while preserving the blood flow to the surrounding branches. In other words, this aneurysm tends to sit between the branches of the kidney arteries and the branches that go to each leg, so while we exclude flow into the aneurysm, we have to maintain flow into those branches. Otherwise, your kidneys would fail and you'd have no blood flow through your legs.” “Traditionally, this was done with open surgery where we would sew a plastic tube below the kidney arteries and above the aneurysm and then down to each leg artery. However, over the last 15 to 20 years, we've advanced this technology where we can do the procedure endovascularly. In other words, we can do it through remote puncture sites at the level of the groin and deploy a device that looks like what I've just described, but instead of going inside the belly directly, we deploy it from the inside of the vessel, and this will minimize the risks of surgery itself.” “Patients who get the latter procedure will often go home the next day. Patients who get the former procedure will generally be in the hospital for five to seven days, and there are higher risks to that. I would suggest that 90 to 95% of the patients that we perform these procedures on now have the less invasive procedure. However, some patients who are not candidates for that less invasive procedure.
