It requires pressure inside to maintain that shape, this pressure is provided by a fluid called as aqueous humor that is produced continuously. Aldo aqueous humor provides nourishes and gives nutrients to the eye tissues such as iris lens and others so this is replaced continuously and should exit or go out continuously, this is how the eye pressure in the eye ball is maintained. However if for some reason the intraocular pressure goes up, it is called glaucoma and it can damage the optic nerve thereby leading to loss of vision. So if the pressure goes up, what should we do in the ball or balloon we have to create an alternative channel for the fluid to go out or clean the fluid whatever channel is left inside. Creating an alternative channel in the wall of the eyewall is called as trabeculectomy. This is the commonest and the most commonest procedure that is performed all over the world. However the entire wall has to be cut to create the alternative channel. Then came in what are called as non penetrating procedure of which canaloplasty involves using a catheter inside the schlemm's canal. Schlemm's canal is the portion where the aqueous humor drains out and goes out of the eye. It is like a major water canal. We just insert a catheter into the schlemm's canal and enlarge the size of the schlemm's canal and create micro perforations so that the aqueous humor can flow out easily. Though this procedure is less invasive, remember it is less successful when compared to a trabeculectomy.
