REACH Fellowship Graduation Ceremony || July 15, 2025

The REACH Fellowship is a 6-month program that seeks to increase access to networks and resources among leaders from communities at the highest risk or burden of chronic diseases, who are innovating food systems change. The REACH Fellowship was modeled after the Nourishing Power Fellowship. The Swetland Center and Catalytic Change Link engaged four teams, each with a grower or nutrition educator and a clinical partner, to tailor produce prescription programs to increase reach among those at the highest risk or burden of chronic diseases in Cuyahoga County. Each team co-developed a program to pitch at the REACH Fellowship Ceremony, aiming for implementation at a clinical site soon thereafter. The fellowship seeks to expand and sustain produce prescription programs that enhance food freedom, agency, and dignity in food traditions, promote wealth building among communities at the highest risk or burden of chronic diseases as program implementers, and achieve healthcare savings for providers and insurers.
