Toric 1-piece IOL in presence of large capsulorhexis run: Using posterior capsulorhexis to stabilize

This cataract patient had 3 diopters of astigmatism (axis 170 degrees) with a planned Toric (5D cyl) in the bag. The surgeon ran the capsulorhexis out and this resulted in very large capsulotomy. The concern here is that the single-piece lens implant may not be stable which could result in decentration, dislocation, and possible iris chafing (UGH syndrome). A single-piece should also not be placed in the sulcus. An option was 3-piece non-Toric but here's a solution. We made a PCCC posterior capsulorhexis, and placed the optic thru the PCCC (posterior optic buttonhole = POBH) with the haptics remaining in the bag. This provides excellent stability and centration.
