In this concise Physioclassroom neurology video, we explain why even a tiny stroke in the brain’s internal capsule can cause devastating paralysis. The internal capsule is a deep, V-shaped bundle of white matter carrying the brain’s main motor and sensory fibers. A small infarct in its posterior limb (often a lacunar stroke) cuts off all motor output to one side of the body. This causes a pure motor hemiparesis, meaning the face, arm, and leg on the opposite side are all weak. In extreme cases the side is completely paralyzed – a phenomenon called dense hemiplegia (essentially all motor signals are blocked without cortical symptoms). We also include a brain CT scan image highlighting a posterior-limb internal capsule infarct. (Tiny lacunar strokes can be hard to spot on CT, so this clear example is instructive.) Seeing the lesion in the posterior limb helps link anatomy to symptoms: because the corticospinal tract runs there, a lesion causes contralateral weakness of face, arm and leg. In fact, internal capsule stroke is a classic pure motor lacunar syndrome, with paralysis on one side and typically preserved consciousness.
