Paradigm Shift in Full-Arch Rehabilitation of Atrophic-Edentulous Maxillae and Jaws

Modern implantology is simplify therapeutic approaches to atrophic and edentulous maxillae and mandibles introducing a new implanto-prosthetic rehabilitation approach. This method succeeds in combining the criteria of mini-invasiveness and biomimetics and is comfortable for both clinician and patients. The principle guiding this approach is the demand coming from the scientific community that advocates increasingly less invasive methods burdened with fewer risks. All to increase patient safety and comfort as well as reduce the time and cost of rehabilitation. While guided by valid concepts, reconstructions of lost bone tissue through grafting have reported uneven results. Furthermore, in the last decade there is an increasing tendency to exploit residual bone with some widely used and literature-validated techniques, such as zygomatic or pterygoid implants. However, even these solutions are not immune from complications. The major objections about the use of such implants is the strong operator-dependence and the preference of mechanical over biological principle. Short (+6 mm to -10 mm) and ultra-short (+6 mm) implants, retain the same functional capacity as standard implants and thus allow us to be able to introduce a new rehabilitative approach to maxillary and mandibular atrophies. This approach presents twelve possible clinical scenarios, eight of maxillary atrophy and four of mandibular atrophy. These are “borderline” scenarios that allow the potential of the procedure to be taken to extremes. In fact, in cases where more bone availability is found, implants of different sizes can also be adopted. It must be kept in mind, however, that placing a longer implant is not directly related to greater treatment efficacy, in terms of establishing and maintaining osseointegration, so simplify clinical scenarios is really possible and is the main direction of modern implantology.
