A pilomatricoma (aka pilomatrixoma, calcifying epithelioma of Malherbe)) is an acquired, benign growth most commonly seen in younger females (pre-teen to teenage). They are usually associated with the brow when they present to an ophthalmologist. A subciliary or supraciliary brow incision can be used to approach these lesions. Complete excision is the goal and is curative. If you have a young female who comes to you with an acquired subcutaneous nodule close to the brow, it will likely be a pilomatricoma. These lesions are most commonly seen in pre-teen and teenage females. They are usually located close to the brow cilia. In this case, a supraciliary incision with a 15 blade is used in the area of the lesion. Westcott scissors are then used to sharply and bluntly dissect around the tumor. The lesion is relatively superficial as it arose from a hair follicle matrix cell. Dissection should be performed around the lesion and care should be taken to not compromise the lesion, as complete excision is the goal. The lesion has the typical white appearance of a pilomatricoma. The mass is carefully engaged with forceps – pressure should be gentle to not disrupt the pseudo-capsule around the lesion. After the pilomatricoma is prolapsed through the incision, additional dissection is carried out around the lesion and the posterior extent of the lesion can be dissected free. The incision is then closed with interrupted 5-0 fast absorbing sutures in a vertical mattress fashion.
