https://DrBalikian.com Today in our neck lift series, we’re going to take a closer look at a powerful and often under-discussed part of the procedure: submandibular gland reduction. This technique plays a major role in achieving those sharp, snatched jawlines that many people are looking for. If you’re aiming for clean angles and a sleek neck contour, this step is often a key part of the overall strategy. So what exactly are the submandibular glands? These are salivary glands that sit deep in the neck, in the fourth and deepest layer of the neck's anatomy. They’re responsible for producing saliva, but they’re not the only glands doing that job. We also have parotid glands near the jawline and sublingual glands beneath the tongue, along with smaller glands inside the mouth that all contribute to keeping it moist. Even if we removed the submandibular glands, which we don’t, you’d still produce more than enough saliva. We’re not removing the entire gland, but reducing the portion that has enlarged or sunk lower in the neck due to aging and gravity. This sagging part can ruin an otherwise perfect result if left untreated during a neck lift. You can’t just put stitches in and pull the glands back up. After years of experience, we’ve found that doesn’t work; they just fall back down. The solution is to safely reduce about 20% of the sagging gland, which can make a huge visual impact, especially when paired with a comprehensive neck lift. If we don’t address these glands, they can look worse after surgery. Tightening the neck without addressing sagging glands can leave two visible lumps under the jawline. I’ve had patients come to me after surgery elsewhere, saying, “What’s this extra fat under my jaw?” Unfortunately, I have to tell them, “That’s your submandibular glands, and they should’ve been reduced during the original surgery.” Why don’t all plastic surgeons do submandibular gland reduction? Many lack the experience or training to safely work in that part of the neck, which contains important structures, including nerves. As a head and neck surgeon with 25 years of experience, I’ve spent much of my career working precisely in that space and understand the safest ways to reduce the glands. Some surgeons warn of nerve injury, but in my practice, I’ve never had a nerve injury from gland reduction because we use advanced instruments like Ligasure or high-precision bipolar devices, minimizing trauma and thermal damage, ensuring safety and effectiveness. In summary, not every patient needs submandibular gland reduction during their neck lift, but when I see that it’s necessary, I’ll let you know. If it’s important, it’s because it’s essential for achieving a sharp, balanced neck contour.
