So only thirty percent of the rhinoplasties I perform are secondary rhinoplasties or tertiary rhinoplasties where patients have had problems with their previous surgery. I spent a lot of time with patients talking about this. We do computer imaging, we do a lot of talking about their problems, identifying the specific problems. My planning is meticulous and we've been really successful, I've been really successful with the results and I've gotten in patients because of that. It's a robust part of my practice. This is a young lady was getting ready to go to college who had rhinoplasty earlier in the year and was very unhappy with her tip and shape of the nose. After much convincing of her mother, she got to my office and I was excited with what I was able to offer and do for her. I was able to completely redo the tip of her nose and gave her pretty much what she always wanted. I think you can even see in this picture how happy she is. The tip is raised, it’s narrower and it’s more in balance with her face. We're going to go to a second image: this one is from the side view and on this one you can see how the nose was raised up, the tip was refined and just looks more balanced and it makes her chin look better, it makes her cheeks look better and the balance of her forehead and even eyelashes look better. There's one final view we have of her which is the oblique view and on the oblique view you can see that she that has the tip again raised up you can see the tip looks separate, it looks nice it looks small it looks refined and it looks like it matches the rest of her face. In this young lady, you can see from the side views on the left she had a rhinoplasty before. She's a nice looking young lady but the tip isn't very good. And people ask me sometimes that means the doctor messed up or did a bad job? Look this isn't easy surgery. Rhinoplasty is very sophisticated and it's hard to get great results. Fortunately she came in we were able to get a great result. I was able to reconstruct the tip of her nose using her own cartilage. We didn’t have to add any specialized cartilage or any implants. We didn't have to do this through an open rhinoplasty. I was able to get a tip that matches her chin, her lips, her plucked eyebrows, her well kept hair. This is a woman who definitely dresses up frequently and looks very well-kept. A sharp nose looks really appropriate on her face and with her features. This side view you can see this patient has had nasal surgery before. Her dorsum isn't very attractive and the tip is really poorly defined. It just looks kind of like a blob on her face. I apologize for the pictures but they were taken the day of surgery when she has a shower cap on. After the surgery you can see I used her own cartilage again and I was able to lower the dorsum, the top part of the nose and make the tip much more refined and sharp using her own cartilage. So altogether it just makes her look pretty again and look more refined. And this is pretty much what she wanted and she is very happy. With meticulous planning and execution, you can get the result that you always wanted.
