Deep Anterior Lamellar Keratoplasty

When pathology is confined to the anterior layers of the cornea, a partial thickness keratoplasty is preferred to a full thickness keratoplasty as long term survival is better and rejection rates are lower. This video shows my deep lamellar keratoplasty technique used in a keratoconic patient. I use a combination of air and balanced salt solution (BSS) to help achieve a deep dissection. If a big bubble is achieved, I then dissect off the residual stroma to expose decemet's membrane. If the big bubble is not attained, then the dissection is usually deep enough to proceed with suturing a lamellar button without the need to convert to a full thickness graft.
