I think that performing the divide and conquer technique of nucleo-fractis during cataract surgery is great and there are some advantages with this procedure, particularly for a beginning surgeon. There is a reason why we start teaching our residents divide and conquer as their first technique for nucleo-fractis. This technique is straightforward and it removes the time limit that we find in phaco chop. With divide and conquer there is no critical window of time to sculpt the grooves and crack the nucleus into quadrants. With phaco chop, we have a time constraint once we achieve vacuum holding power with the phaco probe because we start to lose that suction with a second of two. We want to achieve the phaco chop when the vacuum holding power is high and therefore we have a limited time, which can be a few seconds with a dense lens but often just one second with a softer nucleus. This resident has performed 300 cases and has very good hands. My advice is to start to learn phaco chop. Indeed, it is time to graduate to stop and chop and then phaco chop.
