Impression: • Heterogeneously wall enhancing sellar/suprasellar lesion compressing optic chaisma & adjacent optic nerve causing distention of bilateral optic nerve sheath & tortous optic nerve as described - likely benign (macroadenoma). Description: • A well-defined lobulated lesion measuring - 2.3 x 2.1 x 3 cm (AP X TR X CC) (~ 7 ml) showing hyperintense signal on T2 & isointense on T1 WI is seen in sellar & suprasellar region. Pituitary gland is not separately visualised. On post contrast images heterogenous enhancement is seen. • No elo intralesional calcification noted within. • Anteriorly lesion is abutting A1 segment of bilateral ACA with maintained flow signal. It is abutting bilateral gyri recti without signal change. Posteriorly lesion is seen to have maintained plane with basilar artery with maintained flow signal. Superiorly lesion is compressing & superiorly displacing the optic chaisma along with adjacent optic nerve without obvious signal change. Bilateral optic nerve sheath appears distended & tortuosity of optic nerve. Inferiorly lesion is causing sellar expansion with no extension into sphenoid sinus. • Laterally lesion is not crossing medial tangent line on right ( Knosp grade 0) & not crossing intercarotid line on left side (knosp grade l) Flow signal of ICA is maintained without obvious luminal attenuation. History: Reduced vision High range of Prolactin. Sequence: T2 , T1 sag T2, T1 cor T1 FS post gd sag, cor Note: Dynamic is not mandatory for macroadenoma.
