The transcrestal sinus lift is used to place implants in patients with little dental bone. Here we show the technique. 00:00 Introduction: Implant placement requires sufficient dental bone. For these cases, the atraumatic or transcrestal sinus lift can be used as we show in this 3D animation from Clínica Médico Dental Pardiñas (http://j.mp/cPardinas). 00:05 Can an implant be placed if there is not enough dental bone? For the correct placement of a dental implant it is necessary to have a sufficient bone volume to place the implant.Sometimes there is not enough bone and it is necessary to create it. For this, several techniques can be used, one of them is the transcrestal maxillary sinus lift. 00:23 What is transcrestal sinus lift? This technique consists of first preparing the dental implant bed in the bone with the drills, reaching the cortical bone that forms the base of the maxillary sinus. With a special instrument, this cortical is fractured in a controlled way to access the sinus and the Schneiderian membrane is raised a few millimeters. The graft material is then introduced through the previously prepared orifice, compacting it slightly within the maxillary sinus. Finally, and if the quality and quantity of bone allow it, an implant can be placed immediately in the area. 01:04 What are frontal cut used for? Another way to access the maxillary sinus is by means of special drills called frontal cut, which allow the cortical bone to be traversed to the sinus in a controlled manner and without damaging the membrane, since these drills prevent its perforation. The rest of the process is exactly the same as described before. 01:55 Another autraumatic sinus lift technique: Another variant of this technique, when it is not possible to place the implant immediately, is to defer the placement of the implant until the bone is completely healed and mature, in approximately 4-6 months.
