Safety Video Series: MV Magic Striker - Enclosed space entry incident

This safety awareness video highlights the tragic enclosed-space accident onboard MV Magic Striker at Chennai Port on 21 December 2021. During pre-fumigation preparation, two shore workers entered Cargo Hold No. 4, without testing the atmosphere and without authorization. Due to a lack of oxygen and the presence of toxic gases, both experienced breathlessness. One of them attempted to come out but seeing his colleague in distress below, re-entered the hold to help. Both fell unconscious and were unable to come out. Despite all attempts to resuscitate them, the worker who re-entered the hold to help, lost his life. Fortunately the other worker was hospitalized and survived. The investigation revealed serious lapses — absence of proper enclosed-space entry procedures, failure to test the atmosphere before entry, inadequate supervision, and lack of PPE. The case reinforces a critical message: enclosed spaces must never be entered without clearance, testing, and trained personnel.
