Patient A4, a woman with a complex gynecological history, presented with indications for total laparoscopic hysterectomy (TLH) combined with bilateral salpingo-oophorectomy (BSO) due to ongoing symptoms and suspected pelvic adhesions. Her symptoms suggested significant pelvic adhesion, leading to pain and complications impacting her quality of life. Given these findings, a multi-step laparoscopic approach was planned to address all issues thoroughly. The procedure began with a TLH to remove the uterus, combined with BSO to remove both ovaries and fallopian tubes. Due to the history of adhesions, adhesiolysis was performed to carefully separate the pelvic organs, minimizing risk of injury to surrounding structures and reducing the chance of recurrent adhesion-related symptoms. Ureterolysis was also performed, which involved delicately isolating the ureter from any surrounding adhesions to prevent obstruction or injury.
