►Hi I’m Dr. Vikas Aggarwal, I’m the Head of Department and Director at the Department of Urology, Uroncology, and Kidney Transplant at Aakash Healthcare. ►Today, we’ll talk about a common problem; kidney stones, around which there are a lot of myths and treatment options available to society today. ►I’d like to discuss a few things regarding this with you: a kidney stone will not cause any pain or discomfort as long as it remains in the kidney, which leads many patients to believe they have no kidney stones as there is no pain. Only when a kidney stone passes through the kidney and gets lodged in the ureter (from where urine is passed towards the urethra) will patients experience acute and intense pain, akin to the pain felt during a delivery or a spasmodic muscle pain. Along with this, people may experience nausea and vomiting, pain in the left or right side of the abdomen dependent on where the stone is lodged, a frequent urge to urinate; repeatedly going to the toilet. ►As far as testing is concerned, we begin with common tests that include an ultrasound (which can’t always determine the presence of kidney stones, nor measure their true size, or detect them if they are lodged in the ureter. Therefore, the most useful investigative test is the Plain CT-Scan with No Contrast, which renders a much more clear picture of what’s happening. ►Now if we talk about treatment, most people know that some stones come out on their own, while some don’t. So what determines this? The size of the kidney stone will either allow it to pass through or not. Normally, 4-5 mm stones will pass through the urethra on their own, maybe with some pain, but the chances of them passing through on their own are quite high. However, if the stones are bigger than 5, the chances of their passing through will also decrease. For instance, a 6mm stone will have about a forty percent chance of passing through on its own, a 7mm stone will have thirty percent chances, and if it’s a 1cm or 10mm stone: the chances (of it passing on its own) are very low. ►Treatment will be based on the size of kidney stone, where is it lodged, and the severity of your symptoms. I have seen many times over the course of my practice; I have fifteen years of experience in urology, that even when two-to-three-millimetre stones get lodged in the ureter, patients keep trying and trying to treat them with medication, to the extent that their kidneys almost fail, with puss forming inside. Some patients believe their stones have been cured or washed out once the pain stops, but this is not the case; the stone is very much there and the kidney stops functioning gradually, and gets damaged. ►Then there are quite a few people who tell me that drinking beer flushes out kidney stones, or even drinking even soda: what’s important to understand is that both of these build up more urine than usual which will require more urination, giving the stones a better chance to flush out on their own; the added pressure will also dilute the stones. ►You should get a proper twenty four hour urine test to clearly determine the issue, why you’re facing it, and then determine what diet plan will be best suited, not just any blanket diet. The most important thing to consider is how much water you’ll need to consume, to ensure your urine is clear. If you’re working outside then three to four litres is enough, if you’re at home then two to three litres is good enough to keep your kidneys hydrated, avoid the precipitation of stones and crystal, and prevent the formation of kidney stones.
