Sometimes permanent teeth are not erupting in the proper direction and are not visible yet in the mouth. In these cases, a procedure called an orthodontic exposure may be required, whereby a window is made in the gum. This is generally done with an oral surgeon or periodontist rather than the orthodontist. A gold chain (for purposes of sterilization) and button can be added to the teeth with an elastic thread to gradually guide the tooth into place. This can take about a year to occur.
