These lesions are essential for learning brainstem localization and understanding how specific midbrain structures correlate with neurologic deficits. We begin by reviewing midbrain anatomy, highlighting the cerebral peduncles, red nucleus, oculomotor nerve (CN III), and dorsal midbrain region. Then, we examine how damage to these structures—often due to stroke, demyelination, or compressive masses—produces characteristic patterns of motor, cranial nerve, and movement abnormalities. Weber syndrome presents with ipsilateral CN III palsy and contralateral hemiparesis due to involvement of the oculomotor nerve and corticospinal tract. Benedikt syndrome adds contralateral tremor and ataxia from red nucleus damage. Claude syndrome combines features of both, causing CN III palsy with contralateral ataxia. Parinaud syndrome, usually due to pineal tumors or hydrocephalus, leads to vertical gaze palsy, light-near dissociation, and eyelid retraction from compression of the dorsal midbrain.
