Note the position of the biceps belly on contraction. A prominent muscle belly (popeye sign) indicates a rupture of the biceps tendon. Distal displacement of the muscle belly indicates a proximal tendon rupture while proximal displacement indicates a distal tendon rupture. The proximal tendon of the biceps is palpable within the bicipital groove while alternately rotating the patient's arm inwards and outwards. The distal tendon of the biceps is palpable within the cubital fossa while asking the patient to alternately pronate and supinate or flex and extend the elbow joint. A proximal tendon rupture may be asymptomatic with little loss of function while a distal tendon rupture typically manifests with pain, swelling, and/or a hematoma within the cubital fossa and weakened flexion and supination at the elbow joint.
