CAD-RADS 0 No coronary artery disease, absence of plaque 0% stenosis No further evaluation required CAD-RADS 1 1-24% stenosis and presence of plaque Risk similar to patients with no stenosis, but indicates early atherosclerosis. Plaque Burden: P1: Mild plaque burden. Plaque limited to 1–2 coronary segments. Usually focal, early atherosclerosis P2: Moderate plaque burden. Plaque involving 3–4 segments. More diffuse than P1, but not extensive. P3: Severe plaque burden. Plaque involving 5–6 segments. Considered high-risk extent. P4: Extensive plaque burden. Plaque in ≥7 coronary segments. Very high atherosclerotic burden → higher event risk. CAD-RADS 1: 1-24% stenosis and presence of plaque Risk similar to patients with no stenosis, but indicates early atherosclerosis.
