People with opioid use disorder and other complex health needs have limited access to high quality health care and evidence-based self-management education. The Chronic Disease Self-Management Program in addiction-focused primary care settings can help patients build the skills to better manage their own physical and mental health needs. During this presentation, Dr. Elizabeth Siantz explores considerations for implementing a chronic disease self-management program in addiction-focused primary care settings, from the perspectives of people in recovery from opioid use disorder and serious mental illness and their primary care providers.
